Preheat the oven to 375°F (190°C). Line a roasting pan with aluminum foil.
Place pig tails in a large pot and fill with water to cover. Bring to a boil, cook for 10-15 minutes, then drain and clean the pot.
Return pig tails to the pot, cover with water again, and add onion, garlic, red pepper flakes, and salt. Bring to a boil and cook until almost falling apart (about 2 hours).
Drain the pig tails and place them onto the prepared roasting pan. Discard or save the stock for another use.
Roast in the preheated oven for 30-40 minutes until the tails pop, sizzle, and the skin browns.
Carefully open the oven door and remove the pan. Let the tails cool before serving.
